  • LE VERE CAUSE DELL'AUTOIMMUNITÁ

    Le malattie autoimmuni non nascono dal nulla e soprattutto non arrivano perché il corpo impazzisce da un giorno all’altro e decide di attaccarsi senza motivo. Dietro, quasi sempre, c’è un terreno che nel tempo si altera, si sovraccarica, perde tolleranza, accumula stimoli e finisce per spingere il sistema immunitario fuori asse. Questo è il punto da capire bene, l’autoimmunità raramente è il frutto di una singola causa. Più spesso è la somma di fattori che, messi insieme, aumentano il carico biologico fino a superare la capacità di compenso dell’organismo.

    Oggi si parla di oltre ottanta malattie autoimmuni conosciute, ma il numero reale probabilmente è più alto. Alcune colpiscono in modo più selettivo un organo, come la tiroide nella tiroidite di Hashimoto o nel morbo di Basedow, altre hanno un’espressione più sistemica, come il lupus o l’artrite reumatoide. In comune hanno un punto centrale... il sistema immunitario perde precisione. Invece di distinguere in modo netto ciò che appartiene al corpo da ciò che è pericoloso, entra in una condizione di confusione immunologica e comincia a reagire in modo inappropriato.

    Per capire perché succede, bisogna fare un passo indietro, il sistema immunitario non lavora isolato, è in continuo dialogo con intestino, microbiota, sistema nervoso, ormoni, metabolismo, mitocondri, sonno, stress, carico tossico e infezioni. Quando questi sistemi iniziano a soffrire in contemporanea, l’infiammazione di fondo aumenta, la regolazione si abbassa e il corpo entra in una modalità più difensiva che riparativa, è qui che il terreno si prepara. Un concetto utile per leggere tutto questo è quello della risposta cellulare al pericolo, la cosiddetta cell danger response (ne abbiamo già parlato ma ve lo rispiego brevemente).

    In pratica, quando la cellula percepisce una minaccia che supera la sua capacità di mantenere equilibrio, smette di privilegiare la produzione efficiente di energia e passa alla difesa. Nel breve periodo è un meccanismo intelligente, il problema nasce quando la minaccia non finisce mai... stress cronico, tossine, dieta infiammatoria, infezioni persistenti, sonno scarso, disbiosi: se questi segnali rimangono presenti per mesi o anni, il corpo non torna facilmente in modalità di recupero, resta in allerta e un organismo che vive in allerta cronica è molto più vulnerabile allo sviluppo di disfunzioni immunitarie.

    Uno dei primi grandi capitoli è l’intestino, parlare di autoimmunità senza parlare di barriera intestinale significa guardare il problema a metà. Un intestino infiammato, permeabile o con microbiota alterato non crea solo disturbi digestivi, crea anche una maggiore esposizione del sistema immunitario a sostanze che non dovrebbero passare nel sangue con quella facilità. Frammenti di cibo mal digerito, tossine microbiche, componenti batteriche e antigeni ambientali attraversano una barriera che non regge più bene. Il sistema immunitario li intercetta, reagisce, infiamma, se questa scena si ripete ogni giorno, la risposta immunitaria diventa sempre più aggressiva e meno selettiva.

    Dentro questo quadro rientra anche un tema spesso sottovalutato, la bassa acidità gastrica. Se lo stomaco non produce abbastanza acido cloridrico, la digestione parte male già dall’inizio. Le proteine vengono digerite peggio, aumenta il rischio di fermentazioni e disbiosi, la barriera contro i patogeni si indebolisce e tutto il tratto digestivo ne risente, è uno dei tasselli che spiegano perché tante persone con autoimmunità presentano anni di gonfiore, reflusso, digestione lenta, sensibilità alimentari e irritazione intestinale prima ancora di ricevere una diagnosi.

    Le sensibilità alimentari infatti spesso non sono il problema iniziale, ma la conseguenza di una barriera intestinale che perde integrità. Quando questo accade, alcuni alimenti diventano bersagli ripetuti e in alcuni casi entra in gioco anche il meccanismo del mimetismo molecolare: alcune proteine assomigliano a strutture del nostro corpo da favorire una risposta crociata. Il sistema immunitario colpisce ciò che percepisce come minaccia, ma nel farlo può iniziare a colpire anche tessuti propri. È un errore di riconoscimento, ma non arriva a caso o per sfortuna. arriva dopo mesi o anni di infiammazione, passaggi intestinali alterati e continua stimolazione immunologica.

    Il secondo grande fattore è il carico tossico. Viviamo immersi in sostanze che il corpo deve neutralizzare e smaltire in continuazione, pesticidi, erbicidi, plastificanti, solventi, metalli pesanti, composti presenti nei cosmetici, nei detergenti, nell’acqua, nell’aria, nei contenitori, nei materiali da contatto. Il fegato, l’intestino, i reni, la bile, la pelle e i mitocondri lavorano ogni giorno per gestire questo carico, ma non tutti hanno la stessa capacità di detossificazione. Alcuni hanno polimorfismi genetici sfavorevoli, altri hanno già fegato congestionato, intestino infiammato o carenze nutrizionali che rallentano questi processi. Quando il carico tossico supera la capacità di smaltimento, aumentano stress ossidativo, infiammazione, danno alle membrane e irritazione immunologica. Muffe e micotossine, per esempio, possono creare quadri molto complessi: stanchezza, nebbia mentale, sinusite cronica, reattività chimica, attivazione mastocitaria, disturbi neurologici e peggioramento della tolleranza immunitaria.

    Lo stesso vale per i metalli pesanti, che interferiscono con enzimi, proteine e segnali cellulari e anche gli interferenti endocrini, come gli ftalati o il BPA, non si limitano a disturbare gli ormoni: possono alterare immunità, barriera intestinale e tono infiammatorio generale. Un terzo nodo è la glicemia instabile e la resistenza insulinica, molti non collegano il metabolismo glucidico all’autoimmunità, eppure il legame esiste. Picchi continui di zucchero nel sangue, eccesso di cibi raffinati, oli ossidati, alimentazione molto infiammatoria e sedentarietà mantengono alto il livello di infiammazione sistemica. Inoltre alterano il microbiota, peggiorano la funzione mitocondriale e aumentano lo stress metabolico. Un corpo che oscilla continuamente tra iperglicemia, iperinsulinemia e cali energetici è un corpo che si infiamma più facilmente e regola peggio anche l’immunità.

    Poi c’è lo stress cronico, che non è un concetto vago, ma una realtà biologica precisa. Stress significa aumento prolungato di cortisolo, catecolamine, infiammazione, peggior sonno, peggior digestione, peggior barriera intestinale, peggior resilienza immunitaria. Significa anche un sistema nervoso che continua a leggere pericolo e a tenere il corpo in modalità di difesa. Non serve aver vissuto solo grandi traumi, anche microstress ripetuti, relazioni difficili, ipercontrollo, allerta continua, fatica protratta, insonnia, carico mentale e senso di insicurezza possono alzare moltissimo il peso allostatico, cioè l’usura cumulativa che lo stress imprime al corpo nel tempo.

    Il sonno e il ritmo circadiano sono un altro pezzo enorme. Dormire poco, male o a orari disordinati non toglie solo energie: altera immunità, ormoni, metabolismo, risposta infiammatoria e funzione mitocondriale. La melatonina non serve soltanto a dormire. È coinvolta anche nella protezione cellulare e nella gestione dello stress ossidativo. Se la luce artificiale la sera, gli schermi, i ritmi sballati e la mancanza di luce naturale di giorno disturbano la fisiologia circadiana, il corpo perde un importante fattore di regolazione e recupero.

    Un altro capitolo decisivo è quello delle infezioni croniche o persistenti. Non si parla solo di infezioni acute evidenti, ma di situazioni che restano in sottofondo e continuano a stimolare il sistema immunitario. Disbiosi intestinale, SIBO, Helicobacter pylori, infezioni dentali o paradontali, virus latenti come Epstein-Barr, infezioni da Lyme o coinfezioni, fino ai post-virali che lasciano una coda infiammatoria lunga. In queste condizioni il sistema immunitario resta occupato, irritato, iperattivo ma inefficiente e più a lungo resta in questo stato, più aumenta il rischio che perda tolleranza.

    Infine ci sono le carenze nutrizionali, sembra banale, ma non lo è, un sistema immunitario che deve regolarsi bene ha bisogno di nutrienti adeguati. Vitamina D, zinco, vitamine del gruppo B, selenio, omega-3 e altri cofattori sono fondamentali per la tolleranza immunitaria, la funzione di barriera, la modulazione dell’infiammazione e la produzione di energia cellulare. Se mancano, il corpo diventa più vulnerabile. Non perché una capsula faccia miracoli, ma perché senza mattoni giusti anche la migliore strategia resta zoppa.

    Alla fine, la questione centrale è questa... l’autoimmunità non va letta solo come malattia del sistema immunitario; va letta come il risultato di un sistema intero che per troppo tempo ha accumulato carico senza riuscire a tornare in equilibrio. Intestino alterato, tossine, glicemia instabile, stress cronico, sonno disturbato, infezioni persistenti, carenze nutrizionali e vulnerabilità individuali non sono elementi separati, si sommano e quando si sommano abbastanza, il corpo cambia linguaggio. Per questo, il lavoro serio non è inseguire un unico colpevole o cercare una bacchetta magica... è ridurre il carico totale, migliorare digestione e barriera intestinale, abbassare l’esposizione tossica, stabilizzare la glicemia, ripristinare sonno e ritmo circadiano, cercare eventuali infezioni croniche quando il quadro lo suggerisce, correggere carenze reali... e soprattutto riportare il sistema nervoso fuori dalla difesa continua. È meno scenografico della promessa “guarisci tutto in tre mosse”, ma è molto più vicino a come funziona realmente la biologia.

Questo è il punto da capire bene, l’autoimmunità raramente è il frutto di una singola causa. Più spesso è la somma di fattori che, messi insieme, aumentano il carico biologico fino a superare la capacità di compenso dell’organismo. Oggi si parla di oltre ottanta malattie autoimmuni conosciute, ma il numero reale probabilmente è più alto. Alcune colpiscono in modo più selettivo un organo, come la tiroide nella tiroidite di Hashimoto o nel morbo di Basedow, altre hanno un’espressione più sistemica, come il lupus o l’artrite reumatoide. In comune hanno un punto centrale... il sistema immunitario perde precisione. Invece di distinguere in modo netto ciò che appartiene al corpo da ciò che è pericoloso, entra in una condizione di confusione immunologica e comincia a reagire in modo inappropriato. Per capire perché succede, bisogna fare un passo indietro, il sistema immunitario non lavora isolato, è in continuo dialogo con intestino, microbiota, sistema nervoso, ormoni, metabolismo, mitocondri, sonno, stress, carico tossico e infezioni. Quando questi sistemi iniziano a soffrire in contemporanea, l’infiammazione di fondo aumenta, la regolazione si abbassa e il corpo entra in una modalità più difensiva che riparativa, è qui che il terreno si prepara. Un concetto utile per leggere tutto questo è quello della risposta cellulare al pericolo, la cosiddetta cell danger response (ne abbiamo già parlato ma ve lo rispiego brevemente). In pratica, quando la cellula percepisce una minaccia che supera la sua capacità di mantenere equilibrio, smette di privilegiare la produzione efficiente di energia e passa alla difesa. Nel breve periodo è un meccanismo intelligente, il problema nasce quando la minaccia non finisce mai... stress cronico, tossine, dieta infiammatoria, infezioni persistenti, sonno scarso, disbiosi: se questi segnali rimangono presenti per mesi o anni, il corpo non torna facilmente in modalità di recupero, resta in allerta e un organismo che vive in allerta cronica è molto più vulnerabile allo sviluppo di disfunzioni immunitarie. Uno dei primi grandi capitoli è l’intestino, parlare di autoimmunità senza parlare di barriera intestinale significa guardare il problema a metà. Un intestino infiammato, permeabile o con microbiota alterato non crea solo disturbi digestivi, crea anche una maggiore esposizione del sistema immunitario a sostanze che non dovrebbero passare nel sangue con quella facilità. Frammenti di cibo mal digerito, tossine microbiche, componenti batteriche e antigeni ambientali attraversano una barriera che non regge più bene. Il sistema immunitario li intercetta, reagisce, infiamma, se questa scena si ripete ogni giorno, la risposta immunitaria diventa sempre più aggressiva e meno selettiva. Dentro questo quadro rientra anche un tema spesso sottovalutato, la bassa acidità gastrica. Se lo stomaco non produce abbastanza acido cloridrico, la digestione parte male già dall’inizio. Le proteine vengono digerite peggio, aumenta il rischio di fermentazioni e disbiosi, la barriera contro i patogeni si indebolisce e tutto il tratto digestivo ne risente, è uno dei tasselli che spiegano perché tante persone con autoimmunità presentano anni di gonfiore, reflusso, digestione lenta, sensibilità alimentari e irritazione intestinale prima ancora di ricevere una diagnosi. Le sensibilità alimentari infatti spesso non sono il problema iniziale, ma la conseguenza di una barriera intestinale che perde integrità. Quando questo accade, alcuni alimenti diventano bersagli ripetuti e in alcuni casi entra in gioco anche il meccanismo del mimetismo molecolare: alcune proteine assomigliano a strutture del nostro corpo da favorire una risposta crociata. Il sistema immunitario colpisce ciò che percepisce come minaccia, ma nel farlo può iniziare a colpire anche tessuti propri. È un errore di riconoscimento, ma non arriva a caso o per sfortuna. arriva dopo mesi o anni di infiammazione, passaggi intestinali alterati e continua stimolazione immunologica. Il secondo grande fattore è il carico tossico. Viviamo immersi in sostanze che il corpo deve neutralizzare e smaltire in continuazione, pesticidi, erbicidi, plastificanti, solventi, metalli pesanti, composti presenti nei cosmetici, nei detergenti, nell’acqua, nell’aria, nei contenitori, nei materiali da contatto. Il fegato, l’intestino, i reni, la bile, la pelle e i mitocondri lavorano ogni giorno per gestire questo carico, ma non tutti hanno la stessa capacità di detossificazione. Alcuni hanno polimorfismi genetici sfavorevoli, altri hanno già fegato congestionato, intestino infiammato o carenze nutrizionali che rallentano questi processi. Quando il carico tossico supera la capacità di smaltimento, aumentano stress ossidativo, infiammazione, danno alle membrane e irritazione immunologica. Muffe e micotossine, per esempio, possono creare quadri molto complessi: stanchezza, nebbia mentale, sinusite cronica, reattività chimica, attivazione mastocitaria, disturbi neurologici e peggioramento della tolleranza immunitaria. Lo stesso vale per i metalli pesanti, che interferiscono con enzimi, proteine e segnali cellulari e anche gli interferenti endocrini, come gli ftalati o il BPA, non si limitano a disturbare gli ormoni: possono alterare immunità, barriera intestinale e tono infiammatorio generale. Un terzo nodo è la glicemia instabile e la resistenza insulinica, molti non collegano il metabolismo glucidico all’autoimmunità, eppure il legame esiste. Picchi continui di zucchero nel sangue, eccesso di cibi raffinati, oli ossidati, alimentazione molto infiammatoria e sedentarietà mantengono alto il livello di infiammazione sistemica. Inoltre alterano il microbiota, peggiorano la funzione mitocondriale e aumentano lo stress metabolico. Un corpo che oscilla continuamente tra iperglicemia, iperinsulinemia e cali energetici è un corpo che si infiamma più facilmente e regola peggio anche l’immunità. Poi c’è lo stress cronico, che non è un concetto vago, ma una realtà biologica precisa. Stress significa aumento prolungato di cortisolo, catecolamine, infiammazione, peggior sonno, peggior digestione, peggior barriera intestinale, peggior resilienza immunitaria. Significa anche un sistema nervoso che continua a leggere pericolo e a tenere il corpo in modalità di difesa. Non serve aver vissuto solo grandi traumi, anche microstress ripetuti, relazioni difficili, ipercontrollo, allerta continua, fatica protratta, insonnia, carico mentale e senso di insicurezza possono alzare moltissimo il peso allostatico, cioè l’usura cumulativa che lo stress imprime al corpo nel tempo. Il sonno e il ritmo circadiano sono un altro pezzo enorme. Dormire poco, male o a orari disordinati non toglie solo energie: altera immunità, ormoni, metabolismo, risposta infiammatoria e funzione mitocondriale. La melatonina non serve soltanto a dormire. È coinvolta anche nella protezione cellulare e nella gestione dello stress ossidativo. Se la luce artificiale la sera, gli schermi, i ritmi sballati e la mancanza di luce naturale di giorno disturbano la fisiologia circadiana, il corpo perde un importante fattore di regolazione e recupero. Un altro capitolo decisivo è quello delle infezioni croniche o persistenti. Non si parla solo di infezioni acute evidenti, ma di situazioni che restano in sottofondo e continuano a stimolare il sistema immunitario. Disbiosi intestinale, SIBO, Helicobacter pylori, infezioni dentali o paradontali, virus latenti come Epstein-Barr, infezioni da Lyme o coinfezioni, fino ai post-virali che lasciano una coda infiammatoria lunga. In queste condizioni il sistema immunitario resta occupato, irritato, iperattivo ma inefficiente e più a lungo resta in questo stato, più aumenta il rischio che perda tolleranza. Infine ci sono le carenze nutrizionali, sembra banale, ma non lo è, un sistema immunitario che deve regolarsi bene ha bisogno di nutrienti adeguati. Vitamina D, zinco, vitamine del gruppo B, selenio, omega-3 e altri cofattori sono fondamentali per la tolleranza immunitaria, la funzione di barriera, la modulazione dell’infiammazione e la produzione di energia cellulare. Se mancano, il corpo diventa più vulnerabile. Non perché una capsula faccia miracoli, ma perché senza mattoni giusti anche la migliore strategia resta zoppa. Alla fine, la questione centrale è questa... l’autoimmunità non va letta solo come malattia del sistema immunitario; va letta come il risultato di un sistema intero che per troppo tempo ha accumulato carico senza riuscire a tornare in equilibrio. Intestino alterato, tossine, glicemia instabile, stress cronico, sonno disturbato, infezioni persistenti, carenze nutrizionali e vulnerabilità individuali non sono elementi separati, si sommano e quando si sommano abbastanza, il corpo cambia linguaggio. Per questo, il lavoro serio non è inseguire un unico colpevole o cercare una bacchetta magica... è ridurre il carico totale, migliorare digestione e barriera intestinale, abbassare l’esposizione tossica, stabilizzare la glicemia, ripristinare sonno e ritmo circadiano, cercare eventuali infezioni croniche quando il quadro lo suggerisce, correggere carenze reali... e soprattutto riportare il sistema nervoso fuori dalla difesa continua. È meno scenografico della promessa “guarisci tutto in tre mosse”, ma è molto più vicino a come funziona realmente la biologia. XO - Patrizia Coffaro
